Lab results: 6 questions

Dr. Weir: thank you for questions 1 to 12. These are the last ones: five real calls, then one yes or no for the twelve standard tests. Each one is a test as the lab printed it and how the chart files it now. Tap Yes or No, add a comment if you want, and press Save. You can come back and change answers any time; the last save wins.

1. Estimated GFR (Non African American)

Printed on lab reports as: Estimated GFR (Non African American), GFR Estimated (African American), GFR Estimated (Non-African American) and 8 more spellings

Results in patient charts: 25

The chart files it as: Nothing yet. Older reports print two eGFR numbers side by side, one labeled African American and one not (the older race-based equation).

Should these be filed as two separate tests, African American eGFR and non-African American eGFR? NO leaves them unfiled.

2. LDL SIZE

Printed on lab reports as: LDL SIZE

Results in patient charts: 32

The chart files it as: Nothing yet. This is LDL particle size in nanometers, and we could not find a standard test name that matches it.

Is it OK to leave LDL particle size unfiled? (If you know what it should be called, write it in the comment.)

3. SED RATE BY MODIFIED WESTERGREN

Printed on lab reports as: SED RATE BY MODIFIED WESTERGREN, Sed Rate, Sedimentation Rate-Westergren

Results in patient charts: 7

The chart files it as: Sed rate by the Westergren method. Plain "Sed Rate", which names no method, is filed the same way.

Is that right for all of these, including the one that names no method?

4. eGFR (cystatin C)

Printed on lab reports as: eGFR (cystatin C)

Results in patient charts: 2

The chart files it as: Today: a code for an old race-based creatinine formula (MDRD, black population). That code does not fit a cystatin C result.

Should these 2 results be filed as eGFR from cystatin C (code 50210-4)?

5. Hematocrit (for ECV)

Printed on lab reports as: Hematocrit (for ECV)

Results in patient charts: 1

The chart files it as: Hematocrit measured on an automated blood count analyzer. You said this holds only if the lab ran it on a CBC analyzer; the report does not say.

Keep it filed as measured hematocrit unless the report shows it was calculated?

6. 12 standard tests

Printed on lab reports as: 12 standard tests

Results in patient charts: 372

The chart files it as: Each is filed as the standard test of the same name: LDL PATTERN: LDL pattern: the letter result, A or B; LDL MEDIUM: Medium LDL particles, as a particle count (nmol/L), not a size; NON HDL CHOLESTEROL: Non-HDL cholesterol (total minus HDL), serum; CHOL/HDL C: Total cholesterol divided by HDL; HEMOGLOBIN A1C: Hemoglobin A1c as a percent of total hemoglobin, whole blood; FSH: FSH, serum; LH: LH, serum. IU/L and mIU/mL are treated as the same number; CO2: Total CO2 (bicarbonate) on a metabolic panel; A/G RATIO: Albumin divided by globulin; BUN/CREATININE RATIO: BUN divided by creatinine; IRON BINDING CAPACITY: Total iron-binding capacity (TIBC); SPECIFIC GRAVITY: Urine specific gravity.

Are all 12 filed correctly? If any is wrong, tap No and name it in the comment.
